Fields mark­ed with * are man­da­to­ry By sub­mit­ting the whist­le­b­lower form, I agree that the law firm rei­chert & rei­chert may use my per­so­nal data (first and last name, tele­pho­ne num­ber, if appli­ca­ble con­clu­si­ons from the note or the appen­di­ces) for the pur­po­ses spe­ci­fied in the data pro­tec­tion infor­ma­ti­on for whist­le­b­lo­wers. I have the right to with­draw my con­sent at any time wit­hout giving reasons, wit­hout affec­ting the lawful­ness of the pro­ces­sing car­ri­ed out so far (Art. 7 para. 3 GDPR). I am awa­re that revo­ca­ti­on is only pos­si­ble in indi­vi­du­al cases for a limi­t­ed peri­od of time. This may be the case, for exam­p­le, if the accu­sed com­pa­ny has to be infor­med of my iden­ti­ty in the fur­ther cour­se of the pro­cee­dings.
